Payments & Fees

Provident participates with most insurance plans and for those plans and as a service to our patients, we will bill those plans on your behalf and we will help you in any way we can to assist you in handling claims.

However, please remember that payment is the patient’s responsibility and that regardless of the filing of a claim with your insurance company, you are responsible to pay for any co-payment, coinsurance or deductible at the time services are rendered.

It is important that you understand your insurance plan’s coverage and benefits and for you to verify that our office is “in-network” for your specific plan. It is also expected that you will pay any remaining balance at the time of services. There may be an additional service charge if payment is not made at the time of services. We are glad to work out a payment plan with you if you are having any financial difficulties.
